  • Repeated no-shows or late cancellations may result in scheduling restrictions
  • Emergency appointments are accommodated on a same-day basis when possible

4. Payment & Insurance

Payment is due at the time of service unless other arrangements have been made. We accept most major PPO insurance plans, credit and debit cards, HSA/FSA accounts, cash, and third-party financing through CareCredit, Cherry, and Proceed Finance.

  • Insurance benefits are estimated based on information provided by your carrier; final coverage is determined by your plan
  • You are responsible for any balance not covered by insurance
  • Financing terms are subject to the third-party provider's own terms and conditions

5. SMS Messaging Program

The Meadowbrook Dental SMS Alerts program provides appointment reminders, prescription-ready alerts, and general practice notifications to patients who opt in. Participation is limited to individuals 18 years of age or older.

If you opt in to receive SMS messages:

  • Messages may include appointment reminders, prescription-ready alerts, and general practice notifications
  • Message frequency varies, typically 1 to 4 messages per month, depending on your scheduled appointments and practice communications
  • Message and data rates may apply
  • You opted in through a form on our website, by requesting SMS reminders while scheduling, or through another documented consent method

Consent to receive text messages is not required as a condition of receiving any treatment or services. By opting in, you agree to receive messages sent using automated technology and confirm that you are 18 years of age or older, that you are the subscriber or customary user of the mobile number provided, and that you are authorized to provide consent.

Opt-Out Instructions
